FOR CHILDREN UNDER 18: I acknowledge that my child may participate in a variety of activities provided at the Arts Council WF and I represent that he or she is physically fit to do so.  I agree to assume full responsibility for any risks, injuries or damages, known or unknown, which might be incurred as a result of participating in the program.

FOR CHILDREN UNDER 18: I give permission for my child to be photographed for publicity and/or archival purposes, including but not limited to use on the Arts Council's website, social media site(s), print and electronic newsletters, regional print and broadcast media. 

REGISTRATION
Please note the above scheduled day and time for your class. All registrations are non-refundable.  A registration may be transfered to another class at the discretion of the Arts Council WF staff. 

CANCELLATIONS
In the event a workshop must be cancelled, we will contact the adult email listed above.  Cancelled classes will be rescheduled or refunded, at the discretion of the Arts Council staff.

The Arts Council follows the WFISD policy for closures during inclement weather.  If inclement weather occurs on a non-school day, the Arts Council will make determination about closures.  Closures will be posted on our website and social media as soon as possible.

EMERGENCY CONTACT
In the event of an emergency during class, we will contact the adult emergency contact listed above. 

SIGN IN/OUT
All children under the age of 18 must be signed in at the front desk.  Children under the age of 12 must be accompanied by an adult when signing in.  Children 12 and up may sign themselves in, but must do so before attending class.
